In MyTime, the lead intake form enhances your lead capture process by allowing you to add tailored questions that gather critical information beyond basic contact details, enabling more meaningful insights.
Things to know:
- A Lead Management subscription is required. Contact your account manager or email support@mytime.com to add this module to your account.
- Creating and customizing lead intake forms requires the "Manage Client Setup" access control setting and a hidden setting that must be enabled by the MyTime team. To activate it, contact support@mytime.com.
- Lead intake forms can include only custom client fields. For instructions on creating custom client fields, click here.
- You may add default client fields or create additional custom fields based on the information you need.
- Descriptions can be added to custom fields, which display both in-store and online immediately before the field.
- Any field can be marked as required.
- All information submitted through the form is securely stored in the lead’s profile.

How to Create a Lead Intake Form
Once all necessary custom fields have been set up:
- Go to the Clients tab
- Click the Intake Form link at the top of the screen
- Scroll to the "Lead Intake Form" section and select "Create New Intake Form"
- Enter a name for the form (e.g., Lead Contact Form)
- Click Save
Adding Fields to the Intake Form
Once the form is saved, a list of available custom and default fields will appear on the left side of the screen.
- Check the boxes next to the fields you want to include. Selected fields will appear in the Lead Intake Form preview on the right.
- Click the dropdown arrow beside a field to:
- Add a description
- Mark the field as required
The form preview updates automatically to reflect your changes.
