Making client intake forms available online allows your business to collect important client information before an appointment is booked. This ensures a more efficient and personalized experience. In this article, we’ll explain how clients interact with intake forms on the booking widget and express checkout page.

Custom fields included in intake forms will appear on the booking widget and express checkout page.
To make an intake form visible online, it must be associated with one or more services, classes, or memberships.
If any fields on the intake form are marked as required, clients must complete them before they can finalize their booking.
Once a client completes an intake form, the responses are saved to their profile. Any previously submitted form data will be overwritten to ensure the most current information is stored.
Field descriptions added to custom fields will be shown online and in-store, appearing just before each corresponding field on the form.
To access the client intake form online, your clients should:
- Navigate to your booking widget or express checkout page using your booking link or the “Book” button on your website.
- If your business has multiple locations, the client will first select a location.
- The client will then sign in to their MyTime account, or sign up by clicking Sign Up Now if they don’t already have an account.
- Click Book next to the service, class, or membership they want to book.
- Select a date, time, and staff member (if applicable) using the calendar view.
- On the checkout page, the intake form will appear in the left-hand panel.
- Clients will be prompted to complete the required fields on the form.
- After completing the form, the client will: Enter their payment details, and Click Complete Purchase.
- After booking, a confirmation page will appear, and the client will have the option to print the transaction for their records.
