Providing your family member intake form online allows you to gather essential information from clients about their family members before they complete their booking. In this article, we’ll guide you through the steps your clients will take to fill out the pet intake forms online.
Things to know:
If custom fields are added to an intake form, they will appear online on the booking widget
- To make an intake form visible online, it must be associated with services, classes, or memberships. This ensures that the form is presented to clients when they book a service or class or purchase a membership online.
- If the custom fields in the intake form are set to required, your clients will not be able to complete booking unless these fields are completed
- Information collected on the family member intake form is stored on the family member's profile
- You can add descriptions to custom fields in your intake form, and these descriptions will be displayed on the form both in-store and online, positioned before the field they pertain to.
To access the family member intake form online, your clients should:
- Navigate to your booking widget or express checkout page using the booking URL or book button on your website
- Select the desired location (if the account has multiple locations)
- Sign in if they already have a MyTime user account. If they don't already have one, they can create one by clicking the Sign Up Now option
- Select or add a family member
- Select the "Book” button next to the service they'd like to book
- Proceed to the date and time picker. Choose a date and time and staff (if they are interested in seeing a specific staff). The time picker will show a monthly view, by default
- On the checkout page, the client intake form will be in the left panel on the page
- Enter the card details and then select the “Complete Purchase” button
- On the confirmation page, your clients can print the transaction for their records if they so desire
