Generating Appointment Intake Forms

Appointment intake forms allow you to collect specific information for each appointment and they can be generated from the appointment itself. In this article, we will discuss generating appointment intake forms.   

  1. Navigate to your schedule
  2. Filter by desired location and date
  3. Open an existing appointment
  4. Click on the 'Client Appointment Intake Form' hyperlink        
  5. Select the desired Appointment Intake Form from the drop-down menu. Note: All created intake forms will appear in this drop-down list, even if they are not assigned to the service selected in the appointment modal.                                                                                                             intake form.png
  6. Select the Generate button to open the form in another tab or the Send drop-down menu that will allow you to send it via email, Communicator, or get a shareable link



Appointment Intake Form Generated in Browser 


Once completed, the successful confirmation message will appear
