At MyTime, you can manage the visibility of family member intake form(s) in the "Select an Intake Form" dropdown menu when generating an intake form from the family member profile. In this article, we will provide you with step-by-step instructions on how to do this.

- Hiding family member intake forms helps prevent staff members from sending incorrect intake forms and protects sensitive notes or data about family members from being exposed.
- The option to hide one or more family member intake forms when generating an intake form from the family member profile is controlled by a hidden setting that can only be enabled by the MyTime team. To activate this setting, please contact our Support Team at support@mytime.com.
Hide Family Member Intake Form
Once the hidden setting is enabled:
- Navigate to the Clients Tab
- Click on the Intake Form hyperlink
- Scroll to the Family Member Intake Forms section.
- Locate the intake form you want to hide from the family member profile when generating an intake form, and click "Edit."
- Uncheck the box beside "Show in Generate Intake Form"
- Click "Save"
Once these steps are completed, the intake form will no longer appear in the drop-down menu of the "Generate Intake Form" modal on the family member profile.
